# BUG #23: Task Mode Memory Leak in Epic Execution

**Status:** IDENTIFIED - Root Cause Confirmed
**Severity:** HIGH - Memory accumulation can crash Main Chat
**Affected System:** CFN Loop Task Mode, Epic Execution
**Date Identified:** 2025-10-30

## Summary

Memory leak detected during CFN Loop Task Mode epic execution. Main Chat claude process accumulated 756 MB RSS (and climbing) when spawning multiple agents via Task() tool for AI Organizational Architecture epic Phase 1.

**Process Analysis:**

**Total Memory Impact:**
- Main Chat: 756 MB (expected ~200 MB baseline)
- 15+ agent processes: 500 MB - 2.6 GB each
- **Total system impact: ~15-20 GB memory consumption**

## Root Cause Analysis

### 1. Task() Tool Output Buffering

**Pattern Identified:**
When Main Chat spawns agents via `Task()`, the Task tool **retains full output** from each agent in Main Chat's memory until the agent completes.

**Evidence:**

**Why This Happens:**
- Task() tool is designed for **interactive workflows** with small outputs
- Epic execution produces **large outputs** (multi-file implementations, long analysis)
- Main Chat retains output for display/processing after agent completion
- No streaming/chunking mechanism for large agent outputs

### 2. Epic Multi-Phase Accumulation

**Epic Structure:**

**Memory Growth Pattern:**
1. **Sprint 1.1:** 3 agents spawn → 1.2 MB buffered → Sprint completes
2. **Sprint 1.2:** 3 more agents spawn → 2.4 MB total buffered (additive!)
3. **Sprint 1.3:** 3 more agents → 3.6 MB buffered
4. **Phase 2:** Continue accumulation...
5. **Result:** Main Chat never releases buffered output until **entire epic completes**

**Issue:** Task() tool output retention is **cumulative across epic**, not scoped per sprint.

### 3. Concurrent Agent Context Retention

**Context Structure Per Agent:**

**Memory Impact:**
- Per agent context: ~10 KB initially
- After 2 phases: ~30 KB per agent (previous results accumulate)
- After 4 phases: ~60 KB per agent
- **15 agents × 60 KB = 900 KB just for context objects**

**Plus:** Agent output retention + context = **~2 MB per agent in Main Chat memory**

### 4. Garbage Collection Delay

**Node.js V8 Behavior:**
- Main Chat process runs with default heap settings
- V8 GC triggers at **~70-80% heap usage** (old space)
- Epic execution keeps **active references** to agent outputs (not eligible for GC)
- Result: Memory grows continuously until epic completes or process crashes

**Observed Pattern:**

## Specific Code Causing Issue

### Location 1: Task Mode Coordinator Spawning

**File:** `.claude/commands/CFN_LOOP_TASK_MODE.md`

**Pattern:**

**Issue:** `Promise.all()` collects all agent outputs into array, which stays in memory until coordinator completes entire epic.

### Location 2: Epic Execution Loop

**Pattern:**

**Issue:** Each phase result (5-10 KB) accumulates, passed to next phase, never released.

**Inferred Pattern (Task Tool Implementation):**

## Comparison: CLI Mode vs Task Mode

### CLI Mode (No Memory Leak)

**Pattern:**

- **Coordinator:** ~300 MB (reads Redis keys, not full outputs)
- **Agents:** Independent processes, GC'd after completion
- **Result:** Memory isolated per agent, no accumulation

### Task Mode (Memory Leak)

**Pattern:**

- **Result:** Main Chat buffers all agent outputs, cumulative across epic

## Recommended Fix

### Option 1: Chunked Epic Execution (RECOMMENDED)

**Strategy:** Break epic into smaller Main Chat sessions, commit state between sessions.

**Implementation:**

**Benefits:**
- Main Chat memory resets between phases
- Natural checkpoint/commit boundaries
- User can monitor progress between phases
- If crash occurs, can resume from last completed phase

**Drawbacks:**
- Requires manual Main Chat restarts (not fully automated)
- User must track phase progression

### Option 2: Agent Output Truncation

**Strategy:** Truncate agent output in Task() responses to prevent buffering.

**Implementation:**

**Benefits:**
- Reduces buffered output from ~500 KB to ~200 bytes per agent
- Can still run full epic in single session
- Minimal code changes

**Drawbacks:**
- Loses visibility into agent work (debugging harder)
- Agents must follow strict output format (enforcement difficult)

### Option 3: Hybrid Mode (CLI for Epic, Task for Debugging)

**Strategy:** Use CLI Mode for epic execution, Task Mode only for single-sprint debugging.

**Implementation:**

- Best of both worlds (cost-optimized + memory-efficient CLI for production)
- Task Mode available when debugging needed
- Already implemented (just need documentation update)

**Drawbacks:**
- User must choose mode correctly
- Task Mode still has memory leak for long-running tasks

## Prevention Strategy

### 1. Documentation Update

**Add to `.claude/commands/CFN_LOOP_TASK_MODE.md`:**

**Remaining 0.05 uncertainty:**
- Cannot inspect Task() tool internal implementation (Claude Code proprietary)
- Assumption that output buffering is root cause (not proven via heap snapshot)
- Need production validation of chunked epic execution
